Beyond Hormones: What Urinary Metabolites Reveal That Neither Serum Nor Saliva Can

Translating urinary hormone metabolites into clinical decisions

Most hormone testing answers one question: how much? Urinary metabolite testing answers a more clinically useful one: what does the body actually do with the hormones it makes?

This session is a practitioner-level walk-through of estrogen metabolism, from production to elimination. We follow estradiol through Phase I hydroxylation (the 2-OH, 4-OH and 16α-OH pathways and the CYP enzymes behind them), through Phase II methylation, sulfation and glucuronidation, and out through Phase III elimination, where gut microbial β-glucuronidase activity and bowel transit can quietly return estrogens to circulation. Along the way we look at what shifts these pathways in real clients: methylation cofactors and COMT activity, cruciferous intake, alcohol, insulin, thyroid function, adiposity and environmental exposures.

We close with a short look at progesterone metabolites (5α- versus 5β-pregnanediol, 5α-reductase activity and allopregnanolone) and androgen metabolites (the 5α/5β split of androsterone versus etiocholanolone, and 5α-androstanediol as a marker of DHT drive), and what those patterns explain in clients whose symptoms never matched their serum results.
